EVOLVE WOMEN'S MINDSET MEMBERSHIP - TERMS AND CONDITIONS

Thank you for considering Hanna Carroll, LLC. (“HC, LLC”) to provide services. HC, LLC has developed this agreement (“Agreement”) to meet the following parameters.

APPROACH, GOALS & OBJECTIVES:

This program includes:• One to two 60-minute group monthly sessions • Recordings of sessions are provided • Unlimited WhatsApp support and access

CLIENT ACKNOWLEDGEMENTS:

• As a client, I understand and agree that I am fully responsible for my physical, mental, and emotional well-being during sessions, including my choices and decisions. • I am aware that I can choose to discontinue the membership at any time. • I understand that my experiences may be shared anonymously with other coaching professionals for training purposes or used as marketing of my work. • I understand that video recordings of sessions may be shared with other coaching professionals for training purposes or used as marketing of my work. If client is speaking during video, video will not be shared without client permission. • I understand that coaching is not to be used as a substitute for professional advice by legal, medical, financial, spiritual, or other qualified professionals. I will seek independent professional guidance for legal, medical, financial, spiritual, or other matters. I understand that all decisions in these areas are exclusively mine, and I acknowledge that my decisions and my actions regarding them are my sole responsibility. • I agree to take responsibility for getting what I want from the coaching process. If anything doesn’t work for me or doesn’t “feel right” I will bring it to my coach’s attention immediately so that alternate options can be explored.  • I understand that both myself and my coach will communicate in a direct, honest way, giving clear feedback from a constructive and supportive place. • I understand that this coaching experience is non-refundable.

INVESTMENT:

The investment for the monthly program is $33.33 USD after 1 month of free trial. Client agrees to pay a monthly fee of $33.33 USD monthly and on the same day of the month each month after until membership is cancelled. After cancellation, client will have access to the program until the last day of the billing cycle.

LEGAL:

HC, LLC will make best efforts to accomplish all components of this Agreement but cannot guarantee results. In no event shall HC, LLC be liable in contract, tort, strict liability, warranty or otherwise, for any direct, special, incidental or consequential damages, arising from the consulting services provided by HC, LLC hereunder. HC, LLC is not providing medical, psychiatric, or psychological advice or care of any kind under this Agreement.

[UN]STUCK GROUP MINDSET COACHING PROGRAM ENGAGEMENT AGREEMENT

Thank you for considering Hanna Carroll, LLC. (“HC, LLC”) to provide coaching services. HC, LLC has developed this agreement (“Agreement”) to meet the following objectives:

▪ Provide associated investment for services rendered

▪ Propose engagement parameters

▪ Outline legalities

PROJECT OVERVIEW:

HC, LLC will create a 3 month container from to assist in defining strategic personal and career performance goals, elevating self-awareness, identifying limiting assumptions about self, relationships, money and business, creating sustainable momentum, charging client with an accountability structure, and finding clarity and alignment across mindfulness, wellness goals, life intentions and business/career goals. This is a powerful, comprehensive process for transforming and integrating behavior, skills, and mindset in service to achieving alignment, transformation through remembrance of the self essence, and shifting thought patterns to experience quantum leaps.

APPROACH, GOALS & OBJECTIVES:

Time, consistent effort, communication, stillness and intentional drive are a recipe for making desired shifts professionally, physically, emotionally, spiritually and mentally in this endeavor. HC, LLC will be retained under a standard coaching agreement.

This program includes: • 12, 60-minute group sessions • Daily access to situational spot coaching and community support via What’s App group • “Homework” as given to the group through the Kajabi portal

PROJECT ASSUMPTIONS:

• Neither party has the authority to enter into agreements on behalf of the other party. • All sessions will be conducted in a private place where both parties can be fully present in order to give and receive maximum value

CLIENT ACKNOWLEDGEMENTS

• As a client, I understand and agree that I am fully responsible for my physical, mental, and emotional well-being during my coaching sessions, including my choices and decisions. I am aware that I can choose to discontinue coaching at any time.

• I agree that if I am currently in therapy or otherwise under the care of a mental health professional, that I have consulted with the mental health care provider regarding the advisability of working with a coach and that this person is aware of my decision to proceed with  the coaching relationship.

• I understand that “coaching” is a Professional-Client relationship I have with my coach that is designed to facilitate the creation/ development of personal, professional, or business goals and to develop and carry out a strategy/plan for achieving those goals.

• I understand that coaching is a comprehensive process that may involve all areas of my life, including work, finances, health, relationships, education, and recreation. I acknowledge that deciding how to handle these issues, incorporate coaching into those areas,and implement my choices is exclusively my responsibility.

• I understand that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association. I understand that coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care, or substance abuse treatment, and I will not use it in place of any form of diagnosis, treatment, or therapy.

• I understand that information will be held as confidential unless I state otherwise, in writing, except as required by law.

• I understand that my experiences may be shared anonymously with other coaching professionals for training purposes or used as marketing.

• I understand that coaching is not to be used as a substitute for professional advice by legal, medical, financial, spiritual, or other qualified professionals. I will seek independent professional guidance for legal, medical, financial, spiritual, or other matters. I understand that all decisions in these areas are exclusively mine, and I acknowledge that my decisions and my actions regarding them are my sole responsibility.

INVESTMENT

The investment for the 3 month program is $1500 USD. Client agrees to pay 3 monthly fees of $500 USD on starting before the first coaching session and on the same day of the month each month after. At the conclusion of the initial contract, Client has the opportunity to evaluate results and experiences. If at this time Client wishes to extend their time with HC, LLC, a separate session will be conducted so that both parties can make a fully informed decision as to how to best move forward. At any point within the engagement, if the Client decides that the scope of services should increase to include in-person meetings, offsites, or program development, a mutually agreed upon adjustment will be determined in writing.

I agree to ask for specific support when I need it. I agree to be open to receiving feedback and experimenting with new actions, behaviors, and strategies to create the results I want. I understand that the real work is done in the time between sessions, and I agree to engage fully in the opportunities for expansion offered by homework and experiential learning. I am open to meditating. I understand to expect my coach to respond to email & messages within 24 hours maximum during the business week (M-F). Exceptions for greater response time during weekdays include personal days and holidays. I commit to the future that is possible for me. I affirm that I am fully capable of creating an even more fulfilling, extraordinary, and meaningful life.

CLIENT AUTHORIZATION

By buying this product, you reflect a complete understanding of the services to be provided and the agreements created in support of the Client.

At the conclusion of the initial contract, Client has the opportunity to evaluate results and experiences. If at this time Client wishes to extend their time with HC, LLC, a separate session will be conducted so that both parties can make a fully informed decision as to how to best move forward.
